These Terms of Service ("Terms") govern your use of prosupplyrx.com, the member portal and the products and services offered by ProSupply RX LLC ("ProSupply RX," "we," "us" or "our"). By using the Services you agree to these Terms.
ProSupply RX operates a technology platform that connects patients with independent, US-licensed clinicians and US-licensed pharmacies. We do not practice medicine. Clinical decisions, including whether to prescribe, are made solely by the treating clinician. Nothing on the site is medical advice. In an emergency, call 911.
You must be at least 18 years old, located in a state where the Services are available, and able to form a binding contract. You agree to provide accurate, complete and current information, including a valid government ID and photo when requested for identity verification.
Payment is collected at checkout, before a clinician reviews your intake. Purchasing creates your account and unlocks the medical intake. If a US-licensed clinician determines that treatment is not appropriate for you, nothing is dispensed and the full amount is refunded to your original payment method. A prescription is never guaranteed. Subscription plans renew automatically until cancelled from your member portal before the next billing date. See our Returns & Refund Policy for details.
Use of clinical services requires your agreement to the Telehealth Informed Consent. Some states require a synchronous phone or video visit before a prescription can be written.
Compounded medications are prepared by US-licensed compounding pharmacies for an individual patient. They are not FDA-approved finished products and have not been evaluated by the FDA for safety or effectiveness. Lab results alone are not intended to diagnose, treat or cure any condition and are reviewed by a clinician in the context of your history.
You are responsible for keeping your login credentials confidential and for all activity under your account. You agree not to misuse the Services, interfere with their operation, attempt to access another person's account, or use the Services for anyone other than yourself.
The site, its content, design and trademarks are owned by ProSupply RX LLC or its licensors and are protected by law. You may not copy, reproduce or create derivative works without our written permission.
The Services are provided "as is" without warranties of any kind to the fullest extent permitted by law. Individual results vary and no outcome is guaranteed. To the maximum extent permitted by law, ProSupply RX LLC is not liable for indirect, incidental, special or consequential damages, and our total liability for any claim is limited to the amount you paid for the product or service giving rise to the claim.
Any dispute arising from these Terms or the Services will be resolved by binding individual arbitration under the rules of the American Arbitration Association, except that either party may bring an individual claim in small claims court. Class actions are waived. These Terms are governed by the laws of the state in which ProSupply RX LLC is organized.
We may update these Terms by posting a revised version with a new date. Continued use of the Services after a change means you accept the new Terms. Questions: [email protected].
